► Social Services Director - Stafford Healthcare, a progressive skilled nursing and rehab provider in South King County, is seeking a Social Services Director to join our team in providing care and services to our residents. This person will work closely with our residents in the Medicare Transitional Care and Rehab center, conduct discharge planning, and have a key role with our Resident Care Managers in the compilation of MDS data and reporting. As Director this person manages a team of three other Social Workers who provide coverage for our residents.

Our Social Workers are knowledgeable in Resident Rights and regulations governing nursing homes and Medicaid programs. They work closely with visiting physicians and ARNP's from local hospitals and managed care organizations. The Director is a working manager who oversees the department and the needs and demands for Social Services in our transitional care unit.

If you have a Bachelor's Degree in Social Work or MSW, have long term care experience, are compassionate about seeing people succeed, and want to work full time in a very supportive and challenging environment, we want to talk with you.
